Home for All, with members including key leaders from more than two dozen area service agencies, works continually to improve the current system working with those experiencing or at risk of homelessness. Home for All was instrumental in developing and launching the Coordinated Entry system, which serves as a single point of entry for those facing homelessness.
Additionally, Home for All was responsible for the establishment of Community Care Teams at both Portsmouth Regional Hospital and Wentworth Douglas Hospital, allowing member agencies an opportunity to meet together regularly, triage high risk individuals, and create a wrap around care plan.
Currently, Home for All is working on expanding an existing centralized care management technology solution to the wider service sector, and is developing strategies for early intervention and prevention of evictions.
